Directions for entering a test....

Two copies of both sides of the dogs pedigree.
Print and sign two copies of the executable entry form 1.
The club name and number for the entry form are: JGV-USA (1103)
Check written to JGV-USA for the amount of the test entry fee. (VJP - 85$, HZP - $100, VGP - $150) 4. Send all of the above to the test coordinator.
